- I ACKNOWLEDGE THAT I AGREE TO PARTICIPATE AT MY OWN RISK AND THAT I WILL NOT HOLD ANY PERSON RESPONSIBLE IN ANY WAY FOR ANY PERSONAL INJURY THAT MAY OCCUR DURING MY INSTRUCTION PRACTICE, DEMONSTRATION OR TRAINING OF INTERNATIONAL KARATE DAIGAKU
- ASSOCIATION AUSTRALIA Inc. I ALSO ACKNOWLEDGE SHOULD I DEEM IT NECESSARY IN RELATION TO PERSONAL ACCIDENT/ INJURY INSURANCE, I HEREBY AGREE TO OBTAIN ACCIDENT INSURANCE THROUGH INTERNATIONAL KARATE DAIGAKU.
- I ALSO AGREE TO ABIDE BY THE INTERNATIONAL KARATE DAIGAKU ASSOCIATION AUSTRALIA Inc PLEDGE AND TO NEVER MISUSE THE INTERNATIONAL KARATE DAIGAKU ASSOCIATION AUSTRALIA Inc. IN ANYWAY.
- I ALSO UNDERSTAND THAT INTERNATIONAL KARATE DAIGAKU ASSOCIATION AUSTRALIA Inc. HAS RESERVED THE RIGHT TO REFUSE OR DISQUALIFY MY MEMBERSHIP AT ANY TIME IF I AM NOT OBEDIENT OF THE RULES OF THE DOJO OR OF THE INSTRUCTIONS OF MY INSTRUCTOR.
- I FURTHER CERTIFY THAT I HAVE READ AND I UNDERSTAND THE CONDITIONS OF MEMBERSHIP AND THAT THE ABOVE PARTICULARS ARE TO THE BEST OF MY KNOWLEDGE TRUE AND CORRECT.
- I acknowledge that my membership is automatically renewed annually unless I notify three months in advance. I understand that failure to notify will result in carrying the cost of automatic renewal.
- I acknowledge that passbooks and badges remain property of IKD-Australia and must be returned upon resignation/termination of membership.
- I agree/ disagree to have pictures or videos taken from me/ my child during training and that those images might be occasionally published in the newspaper or on the IKD Facebook and website.
- I acknowledge that this is a binding agreement for per month or per term .
- I agree to pay the monthly/ term fees on time, regardless of whether I attend training or not.
Cancellation policy:
- Your membership is automatically renewed every year and requires a three-month notice, in writing, for termination. This means that by April, you need to cancel your membership prior to the July renewal and new financial year.
- Any later termination will be accompanied by extra fees.
- I acknowledge that the monthly/ term fees are still payable during the 3-months’ notice period whether or not I attend training, during that period.
- Otherwise, the cancellation policy and fee of $ 250 applies.
- I acknowledge that cancellations will not be accepted over the phone or by e-mail.
- I have read and understood the above and agree to all terms and conditions of the agreement.
Privacy:
- You acknowledge and agree that transmissions on the internet are never completely private or secure and you understand that any message or information you send to or download from the Website (where permitted) may be read or intercepted by others.
Governing Law:
- This agreement is governed by, and will be construed in accordance with the laws of Queensland, and the courts of this State shall have jurisdiction in the event of a dispute.
